Swear or Affirm Affidavits /
Statutory Declarations
We do not prepare affidavits or fill statutory declarations. This service is limited to swearing or affirming the document once it’s already been prepared. Only sign infront of the Notary Public/Commisioner of Oaths.
Supplementary documents that are marked as "attached" in the declaration or affidavit should also be present at time of the appointment and will be marked as an exhibit for $7.95 per document.

Certified True Copies
The Notary can attest that copies of documents are true and accurate by comparing to the original. The document(s) you wish to have notarized must be the original, a photocopied document cannot be treated as an original.
For any electronic documents, you will be required to log in during the appointment to validate. Example: Bank Statement / Utility Bill.

CCDC 9A and 9B Statutory Declarations
Form 9A for contractors and Form 9B for subcontractors. Sworn statements that contractors or subcontractors use to submit for payment or to access holdback funds.

Passport Application - In Lieu of Guarantor Package
To apply for a Canadian passport, you need a guarantor who’s known you for at least two years. If you don’t know someone to fulfill that requirement, you’ll need the “In Lieu of Guarantor” form from a passport office. Please make sure everything is filled out except for the signature and the date.

What does it mean to Notarize something?
Notarizing is the act of making documents official by witnessing signatures on contracts, agreements, and other legal documents. By law, you must sign your document in front of the notary for your signature to be notarized. Notaries can also attest that copies of documents are true and accurate by comparing to the original.
Can I Notarize remotely?
Although it is possible to do commissioning work remotely, only in-person services are being offered at the moment. The Notary needs to properly identify you, and watch you sign the document(s) in front of them.
Can I notarize/certify a copy of document?
The document you wish to have notarized must be the ORIGINAL, cannot verify authenticity of a document if it is not an original.
Witnessing Signatures
Making documents official by witnessing signatures on contracts, agreements, and other legal documents. You must sign your document in front of the notary for your signature to be notarized. In-person only - no remote option and photo ID must match name on signature line exactly, no exceptions, even if multiple family names.
Commission of Oaths, Affidavits, Solemn Affirmations and Declarations
A Commissioner of Oaths is someone who is legally authorized to watch you swear or affirm that a document is true. We don’t prepare affidavits, statutory declarations, or solemn declarations. This service is limited to swearing or affirming the document once it’s already been prepared.

Travel Consent Letter Sign/Sealing
The Government of Canada suggests using a consent letter when a child (age 19 and under) is travelling:
- alone
- with only one parent/guardian with decision-making responsibility
- in the care of friends or other family members
- with a group, i.e sports, school, musical or religious

CCDC 9A and 9B Statutory Declarations
The Canadian Construction Documents Committee (CCDC) provides Statutory Declaration of Progress Payment Distribution forms—Form 9A for contractors and Form 9B for subcontractors. They are sworn statements that contractors or subcontractors use to receive payment for the second and later progress payment requests, or to access holdback funds.

Jurisdiction
Documents can be for out-of-province, or out-of-country purposes. Notaries are seen in a different capacity in different parts of the world, in Ontario no legal advice will be given and it is the client's responsibility to confirm local Notarial stamp will be accepted in the jurisdiction it is intended to be used. The sole purpose of the notary will be to attest to a mark being made on the signature line of a document, after verifying government issued photo ID matches the name on the document. Documents can be in a foreign language, you will be asked to print your English name beside your signature.

Agreement | Solid Slate Notary and Document Services

1. Engagement of Services
(a) I, the undersigned (“I,”, “Client”), hereby retain Solid Slate Notary and Document Services (“SSNADS”) and its agents which may include any Notary Public and/or Commissioner for Taking Oaths in and for the Province of Ontario, Canada in their employ (the “Notary”), to provide notarial and/or commissioning services (the “Services”). The Notary represents and warrants that they are in good standing with the Law Society of Ontario.
(b) The Services are to be provided in accordance with:
- Notaries Act, R.S.O. 1990, c. N.6 section 3(1) which permits a notary to:
(1) witness or certify, and attest, the execution of a document;
(2) certify and attest a true copy of a document;
(3) exercise the powers of a commissioner for taking affidavits in Ontario;
(c) Commissioners for Taking Affidavits Act, R.S.O. 1990, c. C.17
(d) Ontario Regulation 431/20, s.2, as it relates to remotely commissioned affidavits and statutory declarations
2. No Legal Advice or Services
(a) I understand and acknowledge that:

I am not being provided with legal advice or legal services; not prior, during or after any interaction with SSNADS, and
-
Documents presented for notarization or commissioning are not protected by paralegal-client confidentiality.
3. Client Identification and Representations
(a) I confirm and acknowledge that:
-
I am the person I claim to be and have provided valid, lawful, government-issued photo identification
-
Government photo identification matches the person named in the document's signatory field exactly if witnessing of signature is required
-
All documents that require Notarial services will only be signed in the presence of the Notary, and not prior
-
Providing false identification or impersonating another person is a criminal offence and punishable by law.
-
I am responsible for providing accurate and complete information to SSNADS.
4. Document Confirmations
(a) I confirm that:
-
The document(s) I wish to have notarized are originals and have not been altered in any way,
-
I will notify the Notary if the document is a photocopy, SSNADS cannot notarize photocopies, must be the original document
-
The document(s) to be witnessed or commissioned are fully completed and will not be altered in any way after the Notary affixes their seal or signature;
-
The document(s) will not be used for any illegal, criminal, or fraudulent purpose(s).
(b) I understand that Notaries are seen in various different capacities throughout the world. In Ontario Notaries can only notarize or commission documents. Documents will not be reviewed for accuracy, completeness and no legal advice will be given at any point.
5. Limitation and Liability
(c) SSNADS and the Notary shall not be responsible for any loss or damage arising from Services provided in good faith, except where caused by gross negligence, fraud, or wilful misconduct.
(d) To the maximum extent permitted by law:
-
SSNADS and the Notary shall not be liable for any direct, indirect, incidental, or consequential damages;
-
Any total liability shall be limited to the amount paid for the transaction in which the Services were rendered;
-
Any legal claim arising from the Services must be commenced within one (1) year of the date the Services were provided.
6. Shipping and Delivery
(a) I consent to SSNADS shipping my documents via FedEx or another courier.
(b) I acknowledge that SSNADS will not be liable for any documents that are lost, damaged, stolen, or delayed due to courier handling, customs, weather, an incorrect address provided by the Client or input error by an SSNADS agent.
(c) SSNADS ships internationally and domestically, fees will be in addition to services offered, any shipping costs to be approved by the Client in writing prior to shipping takes place.
(d) If the cost of shipping exceeds the quoted amount provided via text, email, or phone, I agree to promptly pay the additional fees.
7. Electronic Communication
(a) I acknowledge that email is not a secure method of transmission and may be subject to interception or unauthorized access. I expressly consent to receiving documents electronically by way of email or another predetermined file sharing service at my request.
8. Fees and Payments
(a) Fees are due before or at the time Services are rendered.
(b) Fees are non-refundable once Services have begun.
(c) Additional fees may apply for re-attendance, incidental expenses, printing, courier changes, or other additional requests.
(d) Missed appointments and late cancellations may result in forfeiture of fees paid.
9. Cancellations, No-Shows, and Technical Issues
(a) Appointments cancelled within 24hrs of booked appointment will not be charged.
(b) No-shows are non-refundable.
(c) For remote commissioning, technical failures attributable to the Client (equipment, internet access, connectivity) remain the Client’s responsibility and may not be refunded.
10. Force Majeure
(a) SSNADS shall not be responsible for delays or inability to provide the Services due to circumstances beyond its control, including power outages, internet failures, illness, government restrictions, or third-party service disruptions.
11. Privacy and Data Use
(a) In accordance with Section 4(1) of the Personal Information Protection and Electronic Documents Act (PIPEDA) and Ontario Regulation 431/20 as it pertains to remotely commissioned affidavits and statutory declarations, I acknowledge and agree that SSNADS may:

Retain electronic copies of government-issued photo identification and all documents provided to render or prospectively render Services;
-
Treat “Documents” as both physical and electronic records, images, files, and supporting materials (including exhibits to sworn affidavits);
-
Retain records for a period not exceeding three (3) years from the date Services are rendered, after which, said records will be permanently deleted and will not be retrievable;
-
Store electronic copies of Client documents on an encrypted Microsoft OneDrive folder using AxCrypt, 1Password and Express VPN and by multi-factor authentication;
-
Not share Client documents or personal information with third parties without written consent from the Client, unless required by law or tribunal order.
